On November 9th, Carlson High School will be offering heart screenings for students between the hours of 9 and 4. Parents can follow the link on the QR code on the attached flyer to register their student for a screening. Parents do NOT need to attend the screening as this link contains an online permission form. Students will be released from class for their appointment time and sent back upon completion of the appointment.
These heart checks are $20 per student, however, if you have a financial need and cannot afford it, you can apply for the grant (also within the link). The $20 is due at the time of registration.
